This page includes forms and other
information about our summer camp programs that you need to enroll. They are numbered in a
step-by-step format to ensure that you have all the information you need to
enroll in the camp of your choice.
Step 1: Complete
the enrollment form for your desired camp session. You need to complete a
separate form for each child in the program.
Step
2: Complete
the Camper Health History Form Step
3 (Overnight Camps Only): Complete,
Have Doctor Sign, and Return the Camper Health Care Recommendations. This
form is required if your child is staying overnight at camp for more than 3
nights. Chincapin campers must have this form because of residence week. Step
4: Download forms relevant to the camp your child is attending:
